As a Substitute teacher is an early childhood professional who can implement planned program activities or work as an assistant teacher under the direction of the lead teacher.
Must be able to provide for the health, safety, nurturing and general welfare of the children.
As a Direct Care Provider I am responsible for resident's care, daily living skills, and transportation. Document their resident's progress towards rehabilitation goals. Workers provide direct supervision to individuals in residential programs.
Provide a healthy environment by cleaning cottages and cooking meals. Display appropriate behavior and teach life skills to residents. Provide instruction and evaluation in areas of daily living skills and independent living skills to enhance the resident's ability to reach his/her highest level of independence.
Maintain regular communication with youth, families, and partnering agencies to ensure cases are reported, linked and engaged into care.
Conduct Informal and formal intake assessment, service planning, case coordination, case conferencing, service planning implementation, crises intervention, monitoring and follow-up of services provided.
Offer support provide referrals, identify potential barriers and address any immediate need to help create a seamless process for care engagement. |
